how do you know which side the bppv is on- I understand that you have to
start on one side depending which side the problem is on- how do u
determine that
To determine the side on which you have symptoms you need to lie down quickly with your head turned to one side (ideally with a pillow under your shoulders). If that causes vertigo then you need to treat that side. If there is no vertigo then try doing the same thing looking towards the other side. This is called the DIx-Hallpike maneuver.
Animated Rotary Nystagmus During a Claymation Dix-Hallpike Test for BPPV

@jswensson - I am not sure if you are arguing against something you have
seen in the video or if you are trying to clear something up for someone
specifically. I assume that by periferic you mean peripheral. In that case,
the problem with your statement is that BPPV is a peripheral cause of
vertigo. BPPV causes rotational nystagmus which is what I have demonstrated
above. If you feel that I have made any false statements or representations
please help me clear this up. Thanks for visiting.
@kmedlej - Okay thanks. I am no expert, I only see this problem 1 or 2
times per month in my clinic. I will just say that I intended to depict a
rotational nystagmus which is what occurs in BPPV. In the video, there is
some minimal vertical movement of the eye because I did not use the exact
center of the pupil as the axis of rotation. If you look only at the pupil,
it appears to move mostly vertically, looking at the rest of the eye, you
can see the rotation. Thanks for your input.
@webiocosm The animation correctly shows upbeating torsional nystagmus as
one would expect in BPPV affecting the posterior semicircular canal (about
90% of BPPV). Purely horizontal nystagmus is only seen in BPPV when the
lateral canal is affected. When the anterior canal is affected downbeating
torsional nystagmus is seen. Central causes may exhibit purely torsional or
purely vertical nystagmus.
