Dr. Pauly's recommendation to "determine beforehand" or "negotiate
beforehand" regarding pricing with hospitals is extremely unrealistic for
the average healthcare consumer requiring hospital care, especially in
emergency situations. Saying this situation only applies to a "very small
sliver" of the population is also ridiculous considering the tens of
millions of Americans who still lack private insurance. If you go and see
how much insurance costs on the Healthcare Exchanges and what little you
get for your money, you'll see that costs are still prohibitive, making it
very difficult for many Americans to "follow your dad's advice." This
feels a lot like "picking on the victim" to me, to use your own words.

possible to estimate or negotiate before hand. Many time, additional
activities are to be performed after the initial clinical decision and
estimation. This leads to increase in cost to the patient.
