No, not regarding psychopharmacology, psychoactives (incl. opioids, as evidenced by his attacking messages (incl. stating he hopes the poster dies) referencing false information (e.g. opiates vs. opioids as one example)), nor should he be demeaning to others in trying to get his point across. I know more than him, indisputably more, and I've had friends die of O/D's likewise, but to present his misinformation along with attacking others (wishing they'll dies in an O/D) shows me just how ignorant and "[non]-experienced" he in fact is. @Neelloc1973 (part 4): Meds to help withdrawal symptoms: 1. Ibuprofen for muscle aches. 2. Promethazine for nausea. 3. Benzodiazepines (also potentially habit-forming so they're risky, but can help the anxiety part of withdrawal). 4. Clonidine -- an alpha blocker that helps with agitation and cardiac effects. The latter is risky if you have cardiac issues so you need to talk to your docs. Other meds you are on and other medical problems can complicate the process, as has been the case with me.

I just did my second to last dose jumps. I have gone from 150mg of MS contin (morphine) every 8hrs; the fetinal patch; with daily Dilauded, Percocet, valume, and tramidol thrown in throughout the day. I tried ramping, but I just felt crappy all the time. So I did a few large jumps instead. If you are dependent (not addicted) I would recommend this instead. @Neelloc1973 (part 3): We're all different, but one thing I've learned is that getting off of the patch is harder than from other forms of opiates. Please know that addiction is different from physical dependence, which causes the withdrawal sequelae. If you have a mental addiction it is harder because then you have all the urges to keep using. You may be able at some point to convert to oral medications. If you want to get totally opiate-free you will need an alternate form of pain management.

@Neelloc1973 (part 2): Then you can step down to 175ug/h and go from there. The decreases get harder the lower you go because you are decreasing by a larger percentage of your current dose each time. When you get down to the lower doses, ask for the Mylan brand patches so you can cut them and reduce stepdown amounts to 6ug/h (half of a 25ug/h patch). After each decrease, make sure you're stable before attempting the next. Some docs may want to rush the process but go with how you feel. (cont.)

@Neelloc1973: Hey sweetie. I was on 200ug/h q48h for nearly 10 years so I feel you! First of all yes, your doctor is right: stopping them outright is dangerous; potentially fatal and brutal. I so know how it is to want off immediately but it isn't the way to go. There are a few options, the easiest but longest is to ask your doc to help you by decreasing the dosage. You can do 187.5ug/h for the first taper dose (100ug/h + 75ug/h +12ug/h patches) and stay on that dose for several weeks (cont.)

@Vanorthea: Hey angel! You may want to ask to go back to q48h at the lower dose. Those of us needing q48h dosing will have withdrawal symptoms on Day 3 WITHOUT ANY DOSE DECREASE! You'll be able to tolerate your dose decrease MUCH better w/o having wider serum concentration swings & resulting exacerbation of withdrawal symptoms from extending time b/w patch changes. Also you may wish to ask about clonidine to help w/ the icky effects. Low & slow takes longer but is much more bearable. *hugs*

I have chronic pain and the patch has served me faithfully; I was stable at 200 ug/h for about 9 years. I want to be a mother, so I am volitionally tapering. I have been holding at 42.5ug/h for a bit; I am having another dental surgery tomorrow so I will be staying at this dose for a bit. Are you med-free? How do you deal with your pain now? @Vanorthea: Hey sweetheart! I am stable now on my low dose & will be staying here for a while. I've had some other medical issues coming back so we've halted my taper for now. The most important thing is DO NOT RUSH your taper! That is what I did wrong & why I suffered so much. Sometimes it takes chronic pain pts months to re-stabilize on a lower dose. Also never decrease more than 25% of current dose each time. If you have any more questions, feel free to ask! Good Journey!

I'm so pissed at the DEA scaring docs from doing what is right in pain mgmt & drug seekers/abusers who make our Lived hell. Opiophobia sucks! :( You want to DECREASE your meds, why won't they help? Just wanted to update you since you were so kind to me when I first started this... But tonight at midnight I will be putting on my last 12mcg patch, so in 72 hours I will be taking off the last patch for good. I will then be tapering down on the hydrocodone (7.5/500 tid x 7 days, 5/500 tid x 7 days etc.) It's been a rough journey, but my clarity of mind is better. The pain of course is astronomically bad, but I expected that. Now, to search for non opiate based treatments!

I did ask about it. My pain management doctor refused to help me get off the opiates so I have my primary care helping me. He refuses to prescribe q48 because he keeps claiming that the patch matrix is made so it releases the meds over 72 and he said he won't compromise his license by prescribing a control against the recommend use. I've tried showing him that there are many people that have withdrawal after 48. For me it lasts about 50 hours before I start feeling crappy.

@JamminYamann: Pain Mgmt has come far! Yes, there are longer-acting oral meds, e.g., methadone. There are a vast array of blocks; technological advances which may help (TENS, pain pacer; etc.). Accupuncture, trigger point release, accupressure & PT can also help, as well as desensitization modalities, depending on the type of pain you have. At this point I would recommend you consult w/ either an anesthesiologist or physiatrist (if you aren't yet seeing one of these).

The thing is how do I now stop taking the meds? I wasn't aware of opiate addiction until I tried to stop. I thought I was dying and told my GP what I'd done. @Neelloc1973 (part 5 of 5): If you are eligible and can afford it, there is a procedure called RAD (rapid anesthesia detox) which requires an inpatient stay at a licensed clinic and you are put under anesthesia then forced into acute withdrawal. When you wake up you're through the worst of it. This takes several hours and typically patients stay inpatient for 4 days post-procedure. Good Journey and please keep me posted! XOXOX

Hey there. I took your advice and I'm really glad I did. I was on 50mcg/hr changing every 48. The doctor wanted me to drop down to 25mcg/hr and change every 72. I thought that was way too fast, so we finally compromised on 37mcg/hr and changing every 72. I changed to the lower dose for the first time yesterday, and have the typical chills, sweats, shaking, insomnia etc. I'm thinking of making a video diary to post on here too.

I have the mylan brand patches, so that's a good thing. The only meds I have to help with the withdrawal stuff is the hydrocodone I was using for breakthrough pain before, gabapentin, and imipramine. I actually did ok yesterday, but I just woke up now shaking uncontrollably again. I've been staying in bed mostly just trying to keep warm.

@vanorthea: [continued] Are you on the original Duragesic (gel reservoir) formulation or the Mylan (adhesive only) formulation? Many pts find the Mylan formulation provides superior relief & also makes tapering easier b/c they can be cut for smaller stepdowns at lower doses. Also do you have rescue (short acting) meds?

Gabapentin & hydrocodone will help. I would take both regularly to get you through your Day 3's. I am just appalled that your docs won't work with you more on this. So many of us suffer needlessly! That, to me, is not only a violation of Primum Non Nocere, but unconscionable as well. :(
